Comprehensive Assessment
The first meeting centers on understanding the individual’s needs and situation. During this session, we conduct an in-depth evaluation, including:
- Fall Risk Assessment:
Identifying potential safety hazards and ways to minimize risks.
- Functional Assessment:
Evaluating physical abilities and daily living activities to ensure optimal support.
- Social and Emotional Assessment:
Exploring emotional well-being, relationships, and social support systems.
- Environmental Assessment:
Reviewing the home environment to enhance comfort and safety.

From this thorough evaluation, we will develop a personalized care plan with clear recommendations tailored to the individual’s needs and goals.
Meeting Two:
Care Plan Review and Discussion
The second meeting is dedicated to presenting and discussing the care plan. During this session, we will:
- Review the assessments and recommendations in detail.
- Address questions or concerns to ensure understanding.
- Collaborate to refine and finalize the care plan.
